Cerebral Artery During Nonpulsatile Hypothermic Cardiopulmonary Bypass

چکیده
Background and Purpose We evaluated the utility of blood flow velocity measurements by transcranial Doppler ultrasonography as a tool to indirectly measure cerebral perfusion during cardiopulmonary bypass. Methods We simultaneously measured blood flow velocity in the middle cerebral artery and physiological variables in 18 patients undergoing cardiac surgery under hypothermic cardiopulmonary bypass in which pH and PaCO2 were managed with the alpha-stat acid-base strategy. We expressed blood flow velocity as a relative value of control obtained under normothermia and normocarbia before bypass. We also developed an orginal index, modified cerebral metabolic rate for oxygen, to estimate cerebral metabolic rate for oxygen. Results Relative velocity was significantly (P<.01) reduced during stable aortic cross-clamp compared with before bypass and was significantly (P<.01) increased during rewarming compared with at aortic cross-clamp. Modified cerebral metabolic rate for oxygen significantly correlated with nasopharyngeal temperature during cooling, aortic cross-clamp, and rewarming (r=.756, P<.0001; r=.4, P<.Q1; r=.725, P<.0005, respectively). Calculated temperature coefficient for modified cerebral metabolic rate of oxygen was 2.7 ±1.4 (mean±SD, n=10) during cooling. Only nasopharyngeal temperature and PacO2 were significant determinants of relative velocity during aortic cross-clamp. Conclusions We can monitor cerebral perfusion and metabolism by measurements of relative velocity and modified cerebral metabolic rate for oxygen during hypothermic cardiopulmonary bypass. متن کاملPreservation of static and dynamic cerebral autoregulation after mild hypothermic cardiopulmonary bypass.
BACKGROUND Dysfunction of cerebral autoregulation might contribute to neurological morbidity after cardiac surgery. In this study, our aim was to assess the preservation of cerebral autoregulation after cardiac surgery involving cardiopulmonary bypass (CPB). METHODS Dynamic and static components of cerebral autoregulation were evaluated in 12 patients undergoing coronary artery bypass graft s...
متن کاملCerebral oxygenation during cardiopulmonary bypass in children.
OBJECTIVE Previous work has found cerebral oxygen extraction to decrease during hypothermic cardiopulmonary bypass in children. To elucidate cardiopulmonary bypass factors controlling cerebral oxygen extraction, we examined the effect of perfusate temperature, pump flow rate, and hematocrit value on cerebral hemoglobin-oxygen saturation as measured by near infrared spectroscopy. METHODS Forty...
متن کاملNeuroprotective effect of mild hypothermia in patients undergoing coronary artery surgery with cardiopulmonary bypass: a randomized trial.
BACKGROUND Neuropsychological deficits occur in 30% to 80% of patients undergoing heart surgery and are due in part to ischemic cerebral injury during cardiopulmonary bypass. We tested whether mild hypothermia, the most efficacious neuroprotective strategy found in laboratory studies, improved cognitive outcome in patients undergoing coronary artery surgery. METHODS AND RESULTS Patients 60 ye...
متن کاملGlucose homeostasis. Comparison between hypothermic and normothermic cardiopulmonary bypass.
STUDY OBJECTIVE Disturbance in blood glucose homeostasis during cardiac surgery may cause visceral and metabolic alterations. Hypothermic CPB induces glucose and hormonal changes. As normothermic CPB is used at some institutions, a comparison of blood glucose and plasma hormones between hypothermic and normothermic CPB was performed. DESIGN Prospective nonrandomized study. SETTING Universit...
